“In a five-star hotel in Sao Paulo, Brazil, I was waiting to leave for the airport. With a half hour to pass, I decided to use the hotel gym for some exercise. As I was finishing on the treadmill, my right eye began to swell above the eyelid. I returned to my room and saw that now both eyes were swelling quickly, perhaps from soap on the hotel towels or an allergic reaction to an insect bite.

“I called the Medisys One network from the Brazilian hotel to explain what was happening and to ask for medical advice. I was immediately put in touch with Dr. Sheldon Elman, who recommended that I get help from a colleague as this could be an emergency. I first went to see the hotel concierge, who took me to the local pharmacy. Dr. Elman and the pharmacist spoke over the phone to explore medication options such as anti-histamines and cortisone. In just a short period of time, my entire face had now swollen and my breathing had become restrained. Dr. Elman advised the pharmacist to immediately have me admitted to one of the best hospitals in the area to be treated by a physician.
“Once I arrived at the hospital, Dr. Elman continued to monitor my condition intensively and quarterback treatment over the phone. Not fully understanding the dosage of cortisone that had already been given to me, the physician at the local hospital had wanted to administer intravenous medication, which would have been very dangerous (due to overmedication). Fortunately, Dr. Elman was there to intervene. He spoke to the treating physician over the phone and insisted that the dosage of cortisone already given to me was sufficient; he advised that I should stay at the hospital for surveillance.

“Dr. Elman then followed up with phone calls every three hours during the course of the night (to ensure that I was stable) and at dawn the following morning to verify that my condition had improved and that I was stable enough to return home. Upon my return to Montreal, he insisted on a follow-up visit, at which time he prescribed further medication to counter intolerance to insect bites.
“Dr. Elman’s telephone intervention and calm persistence with the local physicians were essential to resolving a potentially life threatening situation. Even from thousands of miles away, he was able to offer critical medical assistance. By communicating the details of my medical condition and clarifying the precise treatment that had already been administered, he was essential to preventing the dispensation of what would have been a dangerously high dosage of medication.
